The UK does not currently have a high-energy proton beam therapy service.

There is, however, a low-energy proton beam therapy service in Liverpool which can treat tumours of the eye (but cannot treat tumours deeper in the body).

From 2018, PBT will be offered to patients in the UK at UCLH and The Christie NHS Foundation Trust in Manchester. The Government has committed £250 million to fund the two centres.

UCLH’s planning application to build its centre was approved in March 2013 by Camden Council planners. Pending approval by the Greater London Authority, it means building work could start on the centre in spring 2015. The aim is to open the centre in 2018.

Patients who might benefit from high-energy proton beam therapy are currently referred to American or European centres under a Government-funded programme. To date the majority of patients sent abroad have been children.

Some patients who might benefit cannot be treated abroad, however. This is because they are too unwell or there are other issues that make it impractical to travel. With this in mind, the Government has announced plans to invest £250m to support the development of a new national proton beam therapy service.

Proton therapy for head and neck cancer: current applications and future directions - Article

Radiation therapy is a standard treatment modality for head and neck cancer. However, delivery of radiation therapy to areas of disease in close proximity to critical normal structures, can potentially result in severe toxicity. While advances in conformal radiation techniques, like intensity-modulated radiation therapy (IMRT) have led to improvements in the therapeutic ratio, significant treatment-related morbidity still exists. Proton therapy is an emerging and promising treatment modality for head and neck cancers, because of the potential to improve organ sparing and/or safely escalate doses of radiation delivered. Localized radiation therapy to limited areas of the head and neck, such as for a lateralized salivary gland tumor, can be delivered with proton therapy using current techniques. Proton therapy to the bilateral neck, as required for locally-advanced disease, will require the development of intensity-modulated techniques, intensity-modulated proton therapy (IMPT), using pencil beam scanning. Determining the proper role of proton therapy for head and neck cancer should be done in the setting of clinical studies, with careful attention to quality assurance, and meaningful measures of disease control, toxicity and quality of life.
